For the price of an economy-class ticket to London, you could get yourself a fresher and more youthful-looking face if you were to visited your aesthetic doctor and had some Botox and fillers injected into your face. That is something we all know. But what most people do not know is how much advancement the field of Facial Aesthetics has undergone in the last 3-4 years. If you think that aesthetic doctor will simply stick some fillers into your naso-labial folds (the folds beside your nose), then you are quite mistaken. A whole new thinking and approach to rejuvenating faces has taken hold in the world of Aesthetic Medicine.

Traditionally, fillers were used only to make lines shallower. These days however, they can be used to make faces that are flat or broad appear shapelier; or faces that are angular to appear softer. Small chins can be made more prominent; and flat noses more projected. And the list goes on. Even the approach to treating lines has changed. Fillers are used to prop up skin in surrounding areas to stretch out lines like the naso-labial folds without ever injecting into the fold itself. This is the latest approach to using fillers as a sculptor’s tool which new doctors are taught these days. And where fillers ere once a dull and boring cousin of Botox, it now takes center-stage as the most impressive and the most instantly gratifying aspect of any encounter with aesthetic doctors. Not surprisingly, it has had a major revival in popularity in the last 3-4 years.

Fillers rank a close second after Botox as the most popularly requested non-surgical aesthetic treatment on the face. Some 200 million treatments were carried out in the USA alone last year. If you take into consideration Europe, South America, China and the rest of Asia, the number would be phenomenally astronomical.

One could erase 10 years off the face without even missing a day’s work and without any discernable marks. The lunchtime aesthetic makeover was born, thanks in a large part to fillers and botox. “The worldwide aesthetic enhancement industry is now a serious multi-billion dollar industry and fillers have always been a crucial part of this phenomenon, although in recent years, it has as taken on a much more prominent role with the new techniques of facial sculpting and contouring. These new techniques are now the core curriculum for the training of new doctors,” explains Dr David Loh.

But the journey of fillers does not end here. While techniques will no doubt continue to improve, materials will also become longer-lasting and also safer. Today, aesthetic doctors are sculpting faces and shaping skin in such a way that lines stretch out even without a single drop of filler material injected into it. “In the future,” Dr Loh added, “we will probably be looking at how to predict the effects of aging and using fillers to prevent it altogether. Already, studies have shown that fillers alone can stimulate the production of collagen just by their presence in the skin and stretching the skin.”

The future for fillers in Aesthetic Medicine is promising and exciting. The industry is witnessing the beginning to an end of all the problems with inferior quality products that have plagued filler facial enhancements in the past. Fillers are seeing a revival because of the new approaches to sculpting and contouring faces. Undoubtedly, newer and better injection techniques will continue to be introduced in the future, which will transform faces more naturally and for a longer time.

REVIEW: Ulthera Face Lifting Treatment

Ulthera is the first aesthetics device to combine ultrasound imaging with focused ultrasound energy for non-invasive skin lifting. The ultrasound enables visualization of tissue beneath the skin’s surface and Ulthera is the first and only non-surgical procedure to be able to focus energy at prescribed levels. This enables significant lifting of skin tissue while leaving the top layers including the epidermis completely unaffected.

“I’m now able to accurately pinpoint the fibro-muscular layer of the skin and deliver controlled doses of energy onto this layer. Thermage and Titan, the two older generation skin tightening procedures, dumped heat into the entire thickness of the skin, and this resulted in discomfort and were also not always effective,” says Dr David Loh, about this exclusive face lifting machine. There are only 150 machines available around the world and David Loh Surgery is one of the handful to have acquired Ulthera in Singapore.

One of the key features of Ulthera is that it actually triggers the body’s natural healing response which results in regenerated supportive collagen fibres – gradually lifting and tightening skin tissue over time. In clinical trials, 89 percent of patients have noted lifting, tightening and firming in the eyes, general face area, neck, cheeks and along the jaw line. Over a 90-day period, the skin takes on a noticeably refreshed appearance that lasts for 10 months up to a year after the first treatment.

As my face was not sagging yet, I was concerned that I would look too stretched and lifted after Ulthera. Dr Loh reassured me that this would not be the case. “Ulthera is suitable for everyone as the treatment stimulates collagen and makes you look more refreshed. After Ulthera, you will get a more defined jaw line, your lines will get pulled up and collagen stimulated within your skin. In fact, Ulthera will make you look five years younger!”

Treatment process:

Numbing cream is first applied and left on for 20 minutes. This was then wiped off and a cold gel spread liberally over my cheeks down to my neck. Using a handheld device, Dr Loh fired shots section-by-section around my cheeks down to my jaw. He focused on my jaw line and chin to firm those areas and also applied some shots at my temple for my crows’ feet and to lift my eyes.

Ulthera’s ultrasound energy felt like an electrical sewing machine stitching my skin from deep within. Some parts, especially around the bony areas like cheekbones and temples, felt more sensitive. To relieve the pain, I gritted my teeth and focused my thoughts on looking five years younger.

My face felt really tight after the Ulthera treatment and when I tried to smile, my lower face, where Dr Loh focused the ultrasound energy, felt stretched. There was some mild redness which went away in 30 minutes and the tight feeling also eased to a gentler tightness soon after.

The pinch test: Before Ulthera, I was able to pinch the soft and loose skin underneath my chin easily. After Ulthera, the skin underneath my chin firmed up and became more difficult to pinch. Goodbye double chin! Will update again in three months when results peak and I look like a 25-year-old (hopefully!).

1. What is preventive anti aging?

2. At what age does skin start to age?

Your skin actually starts to age immediately after puberty. However, even though your collagen is lessened, it is still able to heal itself quickly. This starts to slow down in the late 30s and 40s where actual signs of aging like sagging and wrinkles appear.

Your lifestyle also plays a big part in your aging process, so if you smoke and sun tan a lot, your skin’s aging will be accelerated.

3. Do genes play a big part in how we will age?

It does. Genetics determine your skin’s healing process. Certain races age better like Asians and African-Americans compared to Caucasians. Your face structure is also important when it comes to aging well. For example, certain features like thick lips can make a person look youthful.

4. I’m only in my 20s, do I need to do any anti aging treatments? Why?

You need to be aware of what environmental damage can do to your skin and start using skin care that protects. Maintanence is better than cure so you can start doing gentle laser and IPL treatments to regenerate collagen and keep your skin in good condition.

5. Will doing these treatments early make my skin more resistant to them as I get older?

No, in fact, it’s been scientifically proven that fillers actually stimulate collagen and improve skin tone. If you test skin before and six months after it’s had filler injections, you will see that the new skin has more collagen.

These treatments actually improve your skin’s baseline and takes it to a higher level so that you age slower.

6. What are some treatments to do for skin maintenance?

You can start with fillers to improve collagen and IPL to take away freckles, reduce acne and acne marks.

7. How do these treatments compare to anti aging creams?

Unlike the treatments above, creams can only go so far. Our skin is an excellent barrier against dust, oils, etc. That’s why sonophoresis machines have been used to drive the molecules of creams into our skin in some beauty treatments. Creams have to be designed very well to be able to deliver nutrients on its own.

Creams have their own place of course – it is comforting to use them when your skin feels dry. It just has to be formulated very well to be able to deliver its benefits.
